Job summary

UnitedHealth Group, through Optum, seeks an RN Care Coordinator to join the Practice Extend Team. You will manage care transitions, coordinate referrals, and perform high-volume outreach to improve patient outcomes.

The role supports a care team including a Clinical Pharmacist and Administrative Coordinator, with telecommuting options nationally and a focus on value-based care initiatives.

Qualifications

• Bachelor's degree required.
• Current, active, unrestricted RN license in a Compact State within the US.
• 3+ years of experience in Case Management and/or Care Coordination.
• Advanced proficiency with Microsoft Excel and PowerPoint.
• Advanced typing speed and ability to navigate Windows environments.

Responsibilities

• Collaborate with Care Team to support patient and provider engagement across transitions, coordination, and medication management.
• Facilitate discharge communications to primary care offices using appropriate tech tools.
• Conduct post-discharge outreach including medication reviews and scheduling follow ups.
• Guide patients to access high-quality, cost-effective referrals for specialists.
• Coordinate appointments for high-risk patients with PCPs and/or specialists.
• Provide patients with benefit information and resources upon request.
• Communicate with provider offices to resolve care issues and facilitate team collaboration.
• Build relationships with participating practices and communicate program goals.
