Job summary

Merakey seeks a Blended Case Manager (BCM) in Philadelphia, PA to develop, coordinate and implement services for assigned consumers under their service plans. The BCM will work across behavioral and physical health needs, family functioning, education and independent living goals.

Earn $23.50 as a BCM

Earn $24.60 as a Bilingual BCM

Position Overview:

The Blended Case Manager(BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains:

• Behavioral and physical healthcare needs
• Family functioning/independent living
• Educational and vocational goals
• Socialization

• Must possess a valid driver's license and have reliable transportation with consistent access to a vehicle during working hours to perform the essential duties of the position, including required travel between work locations.
