Qualifications

• Bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or related human services field.
• Experience in the human services field working with children is preferred.
• Must be bilingual.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Develop, coordinate, and implement services for assigned consumers per their service plan.
• Address behavioral and physical health, family functioning, education/vocational goals, and socialization needs.
• Collaborate with teams and families to ensure continuity of care and progress.
• Monitor and adjust service plans based on consumer progress and changing needs.

POSITION OVERVIEW

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
