Job summary

Merakey is seeking a Blended Case Manager to support clients in Lehigh and Northampton Counties. The role involves home and community visits, coordinating services, and maintaining accurate records to promote stability and independence.

You will work Monday–Friday 8 am to 4:30 pm in a community-based setting, travel to client homes, and receive mileage reimbursement. The position offers competitive benefits and opportunities to grow within a compassionate nonprofit.

Qualifications

• Case Management or similar experience preferred.
• High school diploma with social science credits or a bachelor’s degree in a related field.
• Valid Driver's license and clean driving record; vehicle for travel required.
• Ability to work Monday–Friday, 8 am–4:30 pm.

• Rehabilitation & Consumer Support: assist with living needs and health monitoring.
• Case Management & Service Coordination: serve as primary case manager and coordinate benefits and housing.
• Consumer Tracking & Documentation: maintain accurate service-use summaries and re-authorization paperwork.
• Collaboration & Team Participation: liaise with providers and participate in team meetings.
• Professionalism & Administrative Duties: complete paperwork accurately and on time; on-call duties.

As a Blended Case Manager, you will play a meaningful role in supporting clients throughout Lehigh and Northampton Counties, helping them move toward greater stability, independence, and well‑being. This position calls for strong communication and problem‑solving skills, along with the ability to engage clients directly in their home and community environments.

Responsibilities include:

• Rehabilitation & Consumer Support
  Demonstrate a recovery-oriented attitude, use a consumer-driven and strength-based approach, assist with basic living needs, and ensure annual physicals and medical monitoring.
• Case Management & Service Coordination
  Act as primary case manager, conduct minimal bi-weekly support contacts, complete service/safety plans, coordinate benefits and housing, and follow through with clinical decisions.
• Consumer Tracking & Documentation
  Locate consumers when unreachable, secure past records, develop and update social histories and clinical summaries, maintain accurate service-use summaries, and complete re‑authorization paperwork.
• Collaboration & Team Participation
  Coordinate services with providers, participate in treatment team meetings and service plan meetings with families and providers, act as liaison with supports, and attend all required meetings and trainings.
• Professionalism & Administrative Duties
  Maintain professionalism per policy, complete paperwork accurately and on time, and fulfill on-call duties in the assigned rotation.

This position is community-based and requires travel to homes and other community locations to provide services and support clients. Mileage reimbursement provided.

Earn $20.07 per hour in the full-time (40 hr ) role.

• High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science, and 2 years' experience in public or private human services with 1 year in direct care required or a bachelor's degree in sociology, social welfare, psychology, gerontology, or other social science

• Must be able to work Monday-Friday 8 am - 4:30pm
