Job summary

The role requires a bachelor’s degree in psychology, social work, nursing, rehabilitation, counseling, or related field, and bilingual proficiency. A valid driver’s license and reliable transportation are required.

POSITION OVERVIEW

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
