Qualifications

• Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, or related fields is preferred.
• Registered nurse licensure is acceptable.
• High school diploma with 12 credits in sociology or related field plus two years of direct contact with mental health consumers is acceptable.

Responsibilities

• Develop, coordinate, and implement services per the service plan.
• Serve as primary contact for no more than 30 individuals.
• Provide case-management, service planning, and coordination with other providers.
• Provide supportive counseling and participate in on-call duties.

Position Details

Under the direction of the Blended Case Management Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan. As a member of the BCM team this position will serve as primary contact for no more than 30 individuals. Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients’ needs along with supportive counseling, and will include on-call responsibilities.

The ideal candidate will possess the following qualifications:

• Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
• Registered nurse; or
• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers
