Merakey is seeking a Full-Time Case Manager for the Dual Diagnosis Treatment Team in Dubois, PA. The role involves assessing and planning recovery supports for adults and youth with mental health diagnoses, including autism spectrum, in diverse settings.

• Bachelor’s degree in Social Work, Psychology, or another human service-related field is required.
• Must complete clearances and background checks at Merakey’s expense.
• Valid PA driver’s license with a clean driving record is required.
• Minimum 1 year of experience in the behavioral health field.

Responsibilities

• Assess, plan, and support recovery treatment and plans for adults and youth.
• Provide intensive therapeutic services in community living settings, schools, and other settings.
• Collaborate with the treatment team to implement and transfer skills from recovery plans.

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
