Job summary

Responsibilities include emotional and practical support, data collection, coordinating with birth parents and adoptive families, completing home studies, and post-placement reports.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, nursing, or related human services field required.
• 2 years of professional experience in case management, child welfare, adoption services, or related human services field required.
• A Master’s degree in a related field may substitute for up to one year of experience.
• Proficiency with Microsoft Office (Word, PowerPoint, Excel) and Microsoft Teams; valid driver’s license.
• Bilingual in Spanish preferred (spoken and written).

Responsibilities

• Provide emotional and practical support to expectant and birth parents with a client-centered approach.
• Collect background information, assess needs, and develop individualized case plans.
• Coordinate communication between birth parents and prospective adoptive families.
• Assist in matching, placement, and post-placement supervision of children.
• Complete and document home studies and post-placement reports per agency and regulatory requirements.
• Identify and leverage community resources to support families.
• Travel to client homes and community resources; up to 50% travel.
• Maintain accurate case files and documentation; flexible hours including some evenings/weekends.
