Job summary

LivWell Behavioral in Mesa, AZ is seeking a dedicated Case Manager to guide youth and families from intake through discharge with coordinated, compassionate support. This role emphasizes collaborative care, community resources, and ongoing progress tracking.

You will conduct intake assessments, develop individualized service plans, and maintain documentation while engaging with guardians and school partners.

Qualifications

• Bachelor's degree in social work, psychology, or a related field preferred; relevant experience may substitute.
• Minimum of one year of experience with children with mental health needs.
• Spanish language proficiency strongly preferred.

Responsibilities

• Intake, assessment, and treatment planning for each client.
• Coordinate supportive services for clients and families.
• Communicate weekly with guardians and caregivers on progress.
• Attend quarterly school staff meetings to provide client progress updates.
• Maintain accurate, up-to-date client records and monthly progress reports.
• Provide crisis intervention and support as needed.
