Job summary

LivWell Behavioral in Glendale, AZ is seeking a compassionate Case Manager to coordinate care for youth and families from intake to discharge, delivering individualized, coordinated support in an outpatient setting. You’ll collaborate with families, clinicians, and community partners to ensure timely access to services.

The role emphasizes intake assessments, treatment planning, crisis intervention, and advocacy.

Qualifications

• Bachelor's degree preferred; relevant experience may substitute
• Minimum of one year of experience with children with mental health needs
• Spanish language proficiency strongly preferred

Responsibilities

• Intake, assessment & treatment planning to determine appropriate levels of care
• Coordinate supportive services and communicate progress with guardians weekly
• Advocate for client needs and access to services
• Facilitate Child and Family Team meetings and engage community resources
• Maintain accurate records and complete monthly progress reports per policy
• Provide crisis intervention and safety planning as needed
