Job summary

Lutheran Services Florida, Inc. in Tampa (LSF) seeks a Case Manager Intensive to provide trauma-informed, intensive case management and supportive services to vulnerable clients.

The role involves crisis intervention, safety planning, referrals, and coordinating services in a residential or community setting. Candidates should have a Bachelor's degree, at least two years of case management experience, and strong communication and confidentiality skills; bilingual ability is required.

Qualifications

• Bachelor's degree required with preference for Social Work or related field.
• Two years of case management or social services experience preferred.
• Experience with refugees or immigrants and cross-cultural work is preferred.

Responsibilities

• Provide trauma-informed intensive case management and supportive services to vulnerable clients.
• Conduct weekly in-person trauma support for crisis intervention and safety planning.
• Develop goal-oriented, client-centered self-sufficiency plans.
• Maintain timely communication with supervisor regarding client safety and well-being.
• Prepare detailed documentation in line with LSF policies.
• Coordinate referrals and follow-up services with program participants.
• Supervise and train program interns and volunteers as needed.
• Attend stakeholder meetings and trainings related to trauma-informed care.
