Job summary

Lifeline Community Services in Oceanside, CA is seeking a Case Manager for the Community Services for Families (CSF) program. You will manage a diverse caseload, deliver in-home trauma-informed, family-centered services, and coordinate referrals from CFWB to support parenting goals.

Qualifications

• BA in Social Work, Child Development, Psychology or equivalent plus 2 years of experience with at-risk youth and families.
• Bi-lingual in English and Spanish preferred. Knowledge of Child & Family Development.
• Strong skills in interviewing, rapport building, assessment, goal setting, and motivating youth and families.
• Must have reliable transportation and pass pre-employment background checks.

Responsibilities

• Assist families in developing and achieving parenting goals per CSF case plan using a strength-based, trauma-informed approach.
• Deliver evidence-based parenting curriculums (STEP and/or SafeCare) as directed.
• Perform intakes, assessments, and case management per contract objectives.
• Provide referrals for medical care, insurance, food stamps, nutrition information.
• Participate in team meetings, peer reviews, and report generation.
• Document case files using county forms; produce monthly activity reports.
• Provide services in English and Spanish; interpret documents as needed.
