Devoted Health is seeking an experienced Advanced Practice Provider to join our Transitions of Care (TOC) program in Arizona. The role blends 70% comprehensive initial post‑discharge visits with 30% follow‑up and acute visits, delivering wrap‑around care within a 30‑day window and helping shape the program and EHR enhancements.

Qualifications

• 5+ years of direct patient care with geriatric patients and multiple chronic conditions.
• Experience across acute and chronic care settings and post-discharge transitions.
• Willingness to obtain licenses in required states within 90 days of hire.

Responsibilities

• Perform comprehensive initial TOC visits and post‑discharge assessments.
• Conduct acute/virtual urgent‑care TOC visits and determine the level of care.
• Manage the clinical trajectory across the 30‑day window and adjust plans.
• Participate in interdisciplinary TOC rounds and collaborate with care teams.
• Support build-out of the TOC program and contribute to EHR improvements.
• Adapt to evolving service needs in a fast‑paced environment.
