Devoted Health is seeking an experienced Advanced Practice Provider to build and lead the Medical Transitions of Care program, delivering 30-day post-discharge virtual and in-person care for high‑risk patients. The role blends comprehensive initial visits with timely follow‑ups in a fast-paced, interdisciplinary environment.

Qualifications

• 5+ years of direct patient care with geriatric patients and complex chronic conditions.
• Experience across acute and chronic management and longitudinal care.
• Ability to work full-time, Monday–Friday, 8:30 AM–5:00 PM CST/MST/PST preferred.
• Active RN/APRN or PA licensure; able to obtain required state licenses within 90 days.

Responsibilities

• Perform comprehensive initial TOC visits with full medication reconciliation and care planning.
• Conduct acute/virtual TOC visits and triage to appropriate levels of care.
• Manage the 30-day clinical trajectory and adjust care plans as patient needs evolve.
• Participate in interdisciplinary TOC rounds with care team members.
• Foster safe, open communication and support EHR enhancements and QA projects.
