Qualifications

• High school diploma or GED (minimum).
• Minimum two years of experience in home care coordination or similar role.
• Bilingual Spanish required or preferred.
• Strong communication and teamwork skills.
• Experience with patient care coordination is a plus.

Responsibilities

• Schedule and assign HHAs and PCAs to patient cases.
• Document notes and updates in HHA Exchange; verify staff attendance and schedules.
• Supervise and support HHA/PCA personnel and document information.
• Submit timesheets in HHA Exchange for payroll; assist in scheduling training.
• Make daily calls to patients to ensure excellent service and report incidents.
