Job summary

SCAN Health Insurance is seeking a Registered Nurse - Care Management to deliver high-touch, person-centered care for members with complex medical needs. The RN conducts in-home and telephonic assessments, creates care plans, educates members and families, and coordinates with an interdisciplinary team to improve health outcomes.

The role requires CA RN licensure, bilingual English/Spanish, and travels ~50% of the time in the Los Angeles area; remote/field work is supported.

Qualifications

• ASN required for RN licensure in CA.
• BSN preferred; graduate degree welcome.
• CALIFORNIA RN license required; CCM preferred.

Responsibilities

• Conduct in-home and telephonic assessments and develop care plans.
• Coordinate with interdisciplinary team to ensure safe, timely care.
• Educate members and families on disease processes and self-management.
• Support medication safety and reconciliation with providers.
• Document care plans and progress across systems.
• Manage transitions of care following hospital visits.
• Adhere to CMS/DHCS standards and SNP Model of Care.
