Job summary

Scan-Health-Plan is seeking a field-based RN to provide long-term care management for frail and culturally diverse older adults in Los Angeles County. You will assess, plan, educate, and coordinate services to prevent premature institutionalization while collaborating with a multidisciplinary team.

The role requires a CA RN license, BSN preferred, and 3+ years of home-visit experience. Travel 50%+ is expected; bilingual English/Korean is required.

Qualifications

• Nursing degree required; BSN preferred.
• Registered Nurse with active California license.
• 3+ years in health assessments, care planning, case documentation.
• Experience with older adults, home visits, and mental health/substance issues is desirable.
• Able to travel within LA County and work in a fast-paced environment.
• Bilingual in English and Korean is required.

Responsibilities

• Provide long-term care management to at-risk older adults and caregivers.
• Assess new clients for MSSP program eligibility and certify nursing home eligibility.
• Develop and implement care plans with supervisor approval and client/caregiver input.
• Conduct and document health education for non-clinical staff and families.
• Collaborate with physicians, home health, and community services to arrange services.
• Maintain monthly clinical documentation and purchase of service records.
• Participate in quality assurance and team case conferences.
• Stay updated on Medicare/Medi-Cal policies and training.
