Job summary

PrimaryOne Health is hiring a Nurse Practitioner or Physician Assistant to develop and implement standardized care with supervising physicians, and to address a broad range of health issues in the Columbus community.

The role requires bilingual Spanish skills and an Ohio state certification in the specified specialty, with duties spanning diagnosis, patient education, and record-keeping in a mission-driven setting.

Qualifications

• Must possess an Ohio State certification as a Nurse Practitioner or Physician Assistant in the appropriate specialty (pediatric, family, adult, and women’s health) and shall maintain such certification or eligibility during the term of employment.
• Bilingual in Spanish required.

Responsibilities

• Develop and implement a standard care arrangement with each supervising physician, updated annually, in compliance with Ohio Revised Code.
• Identify, evaluate, and address disease prevention and health promotion issues of the population in the practice while administering quality patient care.
• Work in an independent and interdependent relationship with medical staff, allowing consultation, collaboration, or referral.
• Diagnose and treat acute, chronic, and long‑term health‑care issues.
• Educate patients and/or families about preventive care, medical issues, and use of prescribed medical treatment and/or medications.
• Maintain legible, accurate, and confidential medical records; document evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations.
• Facilitate evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receive and implement directives.
• Occasional travel required.
• Other duties or special projects as assigned.

Essential Job Responsibilities

• Effectively identify, evaluate, and address disease prevention and health promotion issues of the population in the practice while administering quality patient care.

• Be responsible for the diagnosis and treatment of acute, chronic, and long‑term health‑care issues.
• Educate patients and/or families about preventative care, medical issues, and use of prescribed medical treatment and/or medications.
• Maintain legible, accurate, and confidential medical records; document all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with the organization’s regulatory standards.
• Facilitate evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receive and implement constructive directives.
• Occasional travel required.
• Other duties or special projects as assigned.
