PrimaryOne Health is seeking a Family Nurse Practitioner for our Circleville health center. Successful candidates must possess an Ohio State NP certification in pediatric, family, adult, or women's health and maintain certification or eligibility during employment.

Qualifications

• Maintain certification or eligibility during the term of employment.

Responsibilities

• Develop and implement a standard care arrangement with supervising physician, updated annually.
• Identify, evaluate, and address disease prevention and health promotion issues while delivering quality patient care.
• Collaborate with medical staff for consultation, collaboration, or referral.
• Diagnose and treat acute, chronic, and long-term healthcare issues.
• Educate patients and families about preventive care and prescribed treatments.
• Maintain accurate, confidential medical records per regulatory standards.
• Facilitate record evaluation by physician(s) and QA, and implement directives.
• Travel required.
• Perform other duties or special projects as assigned.

PrimaryOne Health is currently seeking a Family Nurse Practitioner for our Circleville health center.

Essential Job Responsibilities:

• To develop and implement a standard care arrangement with each supervising physician, updated annually. In compliance with Section 472343 of Ohio Revised Code.
• Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.
• Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
• Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issue.
• Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatment and/or medications.
• Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with organization’s regulatory standards.
• Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives.
• Travel required.
• Other duties or special projects as assigned.

Successful candidates must possess an Ohio State certification as a Nurse Practitioner in the appropriate specialty-pediatric, family, adult, and women's health, and shall maintain such certification or eligibility during the term of their employment.
