Qualifikationen

Aufgaben

• Complete initial, quarterly, annual, and significant-change psychosocial assessments.
• Complete assigned MDS sections, assessments, and supporting documentation accurately and timely.
• Participate in the MDS process and interdisciplinary care planning.
• Address discharge-planning requirements, including Section Q and follow-up.
• Provide counseling and psychosocial support to residents and families.
• Assist residents with adjustment to institutional living, illness, and disability.
• Coordinate discharge planning and safe transitions to the community or another level of care.
• Identify services, housing, transportation, and resources needed for safe discharge.
