Job summary

Clear Destination Inc. is seeking a Social Services Coordinator to plan, organize, and deliver social services, assess and summarize cases, and provide case management to address psychosocial and socio-economic needs of members and caregivers.

The role collaborates with the Center Director and clinical team to ensure HIPAA-compliant, high-quality care within state regulations, with duties spanning outreach, documentation, and community resources.

Qualifications

• Bilingual in English and Spanish required.
• Degree in Social Work (BS/MSW) required.
• At least 1 year social work experience in a health care facility; seniors/disabilities preferred.
• Knowledge of state and federal environmental and OSHA regulations.
• Strong collaboration across staff levels.
• Excellent verbal and written communication.
• Excellent interpersonal and customer service skills.
• Excellent organizational and time-management skills.
• Thrives in a collaborative, team environment.
• Good computer skills.
• Ability to step in and support the team.

Responsibilities

• Provides case management coordinating with internal and external services for members and families.
• Coordinates with Nurse Manager/clinical team for care plans.
• Participates in Safety and Quality Assurance Committees and schedules Member Council meetings and Caregiver Support Groups; records notes as required.
• Evaluates referrals to determine admission appropriateness and assists with referrals when inappropriate.
• Explores financial situations and assists members with resources.
• Ensures compliance with company policies and regulatory authorities and prepares paperwork.
• Maintains written communication with members and families; facilitates counseling and sessions.
• Maintains accurate files/records of member information; provides billing, transportation, and referral information as needed.
• Participates in community outreach and center marketing activities; engages with health and social service providers.
• Provides active assistance with members (hygiene, meals, ambulation, arrivals/departures, activities).
• This list may be expanded with additional duties as needed by management.
