Old Colony Elder Services, Inc. seeks a dedicated Geriatric Support Service Coordinator to join our Senior Care Options department to help determine eligibility and ensure the provision of appropriate services for eligible members.

Qualifications

• Two years of experience in human services, preferably with elder population.
• Bachelor's degree in social work or related field; current licensure if appropriate.
• Excellent written and verbal communication skills.
• Excellent computer skills and proficiency with Microsoft Office Suite.
• Bilingual Chinese (Cantonese or Mandarin) preferred.

Responsibilities

• Promote independent functioning of members and provide services in the most appropriate, least restrictive environment.
• Perform initial and ongoing assessments of health and functional status; develop individualized care plans.
• Develop rapport with consumers to coordinate, monitor, and adjust care plans as needed.
• Provide regular contact with consumers, including home visits, to ensure progress towards goals.
• Maintain accurate documentation of services and progress.
