Qualifications

• Bachelor’s Degree in Psychology, Sociology, Social Work or related field; additional relevant experience considered.
• 2–3 years of professional experience with at-risk/homeless youth or related services.
• Bilingual English/Spanish; strong communication and inter-agency coordination; trauma-informed approach.

Responsibilities

• Engage youth in developing their Individual Service Plan.
• Create an ISP addressing all critical needs using trauma-informed, positive-youth-development model.
• Collaborate with youth to structure goals with timelines and support.
• Link clients to needed services, referrals, and access facilitation.
• Monitor progress and coach youth to plan and follow through.
• Motivate clients to improve health, mental health, and overall functioning.
• Coordinate between clinical, outreach, and residential teams as a unit.
• Engage youth with case management outside the building, e.g., walks or outings.
• Help youth with resume building, job applications, and interview prep.
• Assist youth in identifying and pursuing educational goals.
• Modify case plans for youths with developmental or health challenges as needed.
• Provide unconditional positive regard regardless of background or behavior.
• Collaborate with team to align activities with youth needs.
• Maintain statistics and complete reports; attend supervision and meetings.
• All other duties as assigned.
