Job summary

WestCoast Children’s Clinic in Oakland, CA seeks a Bilingual (Spanish) C-Change Case Manager to advocate for youth and link families to housing, health, education, and services. You will collaborate with clinicians, parents, social workers, probation officers, and teachers to support youth’s mental functioning and independence.

Qualifications

• Bachelor’s degree preferred
• Bilingual in English and Spanish required
• Experience with case management and rehabilitation services
• Experience with high-need youth and dual diagnosis is a plus
• Knowledge of Alameda and SF County resources for TAY youth
• Willingness to travel within ~90 miles of Oakland office
• Ability to document thoroughly and work in a team setting

Responsibilities

• Provide advocacy, targeted case management, and community-based rehabilitation services to transition-age youth
• Identify and connect clients with housing, medical, education, employment resources
• Perform risk assessments, crisis intervention, and safety planning
• Coordinate with legal resources and support court appearances as needed
• Provide psycho-education about sexual exploitation and related services
• Assist with Medi‑Cal enrollment and regional center services
• Maintain compliance with EPSDT Medi-Cal billing and agency policies
• Deliver services in community settings requiring travel by car
• Demonstrate professional conduct and work within a multidisciplinary team
