Job summary

Mental Health Resources in Clovis, NM is seeking a CCBHC Service Navigator to review intake screeners, educate clients on programs, and connect them with care that fits recovery goals. You will guide clients through referrals to community resources and support services.

Qualifications

• High school diploma required.
• Valid driver’s license.
• Willing to obtain certification in Motivational Interviewing and SBIRT.
• Bilingual/Spanish-speaking encouraged.

Responsibilities

• Meet with clients at intake to review screeners and link to community resources.
• Refer clients to CCBHC programs that meet immediate needs and reduce barriers to care.
• Conduct brief intervention for problematic alcohol use if AUDIT-C indicates.
• Discuss agency and community programs to support recovery needs.
• Perform Health Screenings and collect basic health data (BP, weight).
• Assist clients in creating advance directives and connect to resources.
• May conduct 24-hour follow-up calls after discharge from acute care.
• Document interactions, referrals, and follow-ups in the EHR.
