Job summary

Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to coordinate services for individuals with developmental disabilities in Kern, Inyo, and Mono Counties. You will develop IPPs/PCPs, monitor progress, provide advocacy, and connect families with community resources while ensuring compliance with regulations.

Ideal candidate has a BA/BS in a human services field, two years of relevant experience, and strong communication skills; bilingual (Spanish/English) is highly

Qualifications

• BA/BS degree in human services or related field (social work, psychology, sociology, guidance and counseling, etc.).
• Two years of experience working with developmentally disabled populations or equivalent.
• Bilingual Spanish/English is highly desirable.

Responsibilities

• Provide overall service coordination, advocacy, and services arrangement for consumers and families.
• Develop and implement IPP and PCP, and prepare progress reports and annual reviews.
• Perform follow-up, reassessments and periodic reviews of progress and program effectiveness.
• Identify community resources and information/referral; initiate vendored services and monitor delivery.
• Stay up-to-date with state/federal regulations and agency policies related to consumer services.
