Job summary

Turning Point of Central California, Inc. seeks a Mental Health Specialist/Case Manager IV to provide targeted services to clients, including crisis assessments, case management, and linkage to housing, medical, and educational resources.

You will work in settings such as client residences, schools, and tele-health, documenting in EHR, and collaborating with community partners. A Bachelor's in social work or counseling and Spanish bilingual ability are required.

Qualifications

• Effective communication in written and spoken English.
• Ability to communicate effectively in Spanish and other languages preferred.
• Experience or exposure to mental health services and care coordination.

Responsibilities

• Provide mental health services to clients in homes, schools, community locations, tele-health and clinic sites.
• Document service provision in EHR in a timely manner.
• Coordinate case management to reduce barriers to treatment.
• Collaborate with community partners and county staff as part of mental health activities.
