Job summary

Turning Point of Central California, Inc. is seeking an ACT/FACT Dual Diagnosis Mental Health Specialist/Case Manager V to join the treatment team.

Qualifications

• A bachelor's degree from an accredited or regionally approved college or university in social work, psychology, vocational rehabilitation, criminal justice, or a related field that is directly related to the specific responsibilities of the job.
• At least one year of experience in a mental health or human services related field.
• Current certified training in emergency first aid procedures preferred.
• Preferred California Alcohol and Drug Programs (ADP) approved substance abuse counseling certification or enrollment in an ADP approved course within six months of hire.
• Knowledge of substance abuse, addictions, and related disorders; ability to provide educational groups.

Responsibilities

• Under the supervision of the Supervising Personal Service Coordinator, assist in the accomplishment of the case management plan with clients by screening, evaluating, identifying, and implementing specific case management activities as identified in the treatment plan to support clients in achieving their goals.
• Plan, coordinate, and facilitate services for Severely Mental Ill (SMI) clients.
• Work on a multi-disciplinary team to support clients with an array of needs delivered as an integrated service, including medications, therapy, and groups, housing, employment, benefits, transportation, physical health, and substance use.
• Provide frequent and assertive contact to individuals, primarily community/field-based services with occasional office-based services.
• Provide direct services, including case management and rehabilitation skills to clients.
