Job summary

Turning Point of Central California, Inc. is seeking a Personal Service Coordinator/Case Manager III to assist individuals in accessing Medi-Cal eligibility, medical, housing, and other services.

This full-time role requires coordinating services and delivering direct client support in a day-shift schedule. The position emphasizes bilingual Spanish communication, strong documentation, and collaboration with a multidisciplinary team.

Qualifications

• Bachelor’s degree in social work or counseling
• 1 year experience in a related field
• Bilingual Spanish preferred/strong Spanish communication
• Ability to work with diverse groups and manage case loads
• Experience with EMR systems and Microsoft Office tools

Responsibilities

• Assist in case management plan execution for individuals under supervision.
• Maintain case records per Medi-Cal guidelines and contracts.
• Attend weekly staff and supervision meetings.
• Provide direct services as outlined in the service plan.
• Refer cases to County Mental Health and other resources as applicable.
• Drive personal vehicle for agency business and ensure transportation for clients.
