Job summary

Conviva Senior Primary Care seeks a Care Coach to provide proactive, patient-centered care coordination and social support for the highest risk top 5% patient membership. You are the primary contact for patients and will coordinate with primary care, specialists, pharmacies and home health to reinforce care plans.

This role involves travel to patients’ homes and clinics, a mix of field and clinic work, and requires 3+ years in ambulatory or senior care with bilingual English/Creole proficiency.

Qualifications

• Healthcare professional with 3+ years of ambulatory, primary care, or senior-care experience with direct patient care.
• Bilingual English/Creole required with an English/Creole proficiency assessment.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfort conducting home visits and community-based outreach.
• Experience in patient education, care coordination, and social support of high-risk or geriatric populations.

Responsibilities

• Coordinate care across health and social service systems, serving as patient advocates and clinical supports.
• Conduct structured patient interviews, document findings, and escalate as needed.
• Perform home visits to assess living conditions and safety; review environmental or social factors.
• Identify barriers to care and connect patients with community resources.
• Deliver chronic disease education and reinforce provider recommendations.
• Serve as liaison between patients, primary care, specialists, pharmacies, home health and community providers.
• Support care transitions, coordinate follow-up, and close care gaps.
• Engage in community outreach and maintain cultural competence.
• Participate in High-Risk Rounds and develop holistic patient plans.
