Qualifications

• 3+ years of ambulatory, primary care, or senior-care experience with direct patient care.
• Bilingual English and Spanish (read/write/speak).
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfort with home visits and community outreach.
• Experience in patient education, care coordination, and social support for high‑risk/geriatric populations.

Responsibilities

• Conduct structured patient interviews and document health-related information; share findings with providers.
• Perform home visits to assess living conditions and safety; review environmental/social factors affecting engagement.
• Identify barriers to care; connect patients with community resources and social supports.
• Deliver culturally appropriate chronic disease education using approved materials.
