The role collaborates with medical and behavioral health teams to coordinate services and support ECM participants. The position emphasizes bilingual capabilities (Spanish or Armenian), strong communication, and familiarity with local resources.

Qualifications

• Bachelor of Science in Health sciences, public health, social work, sociology, or related fields required
• Bilingual Spanish or Armenian required
• Excellent written and verbal communication skills
• Knowledge of community resources and government benefits
• Strong advocacy skills
• Experience in health care environment preferred
• Culturally sensitive and able to work with a diverse population

Responsibilities

• Follows up to ensure appropriateness of referrals
• Supports the integrated care process
• Serves as a communication link between medical and behavioral health providers
• Addresses social service needs such as food insecurity, housing, special needs services, substance use, transportation, education and vocational issues
• Arranges, coordinates, monitors, evaluates and advocates for a package of services for ECM Program participants
• Performs other duties to support the organization's mission, vision, and values
