Job summary

San Ysidro Health Behavioral Health Case Manager is sought to provide direct services and coordinate care for patients with complex needs. You will conduct assessments, develop case plans, and collaborate with PCPs/psychiatrists to ensure access to care and timely referrals.

Responsibilities include patient education, documentation in the EMR, and active participation in staff meetings and outreach to improve referral outcomes across clinics and satellites.

Qualifications

• Bachelor's degree in psychology, social work, or a related field.
• High school diploma with a minimum of 2 years of behavioral health case management experience.
• Bilingual Spanish/English required.
• Excellent customer services, communication, and documentation skills.
• Ability to work cooperatively with multiple departments in a large community health center.

Responsibilities

• Provides direct services including case management, referral support, warm handoffs, and psycho-educational groups.
• Conducts comprehensive assessments of patients’ health and psychosocial needs and creates a case management plan with family and staff.
• Develops therapeutic alliance and orients patients to behavioral health services.
• Coordinates patient care with behavioral health providers, PCP, and/or psychiatrist via collaboration and follow up.
• Educates patients and families about treatment options, resources, and insurance benefits to support informed decisions.
• Maintains timely, complete records in the electronic health record system with progress notes and plans.
• Participates in meetings and community engagements to improve referral outcomes and care quality.
• Maintains productivity and adheres to clinic guidelines.
