Qualifikationen

• Engage ECM Members using in-person and tele-health modalities.
• Educate members about ECM benefits and eligibility criteria.
• Outreach to community members through events and programs.
• Accompany members to office visits and advocate with care providers.
• Promote health self-management and monitor treatment adherence.
• Provide health navigation, referrals, and access to community resources.
• Arrange transportation for medical appointments.
• Document all activities and maintain compliance with policies.
• Participate in team meetings and trainings.
• Other duties as assigned.
