Job summary

Nsight Health is hiring a Bilingual LVN/LPN for a remote Remote Patient Monitoring (RPM) role. You will support patients with real-time health monitoring, respond to alerts, and deliver timely life-improving interventions, while educating and empowering patients in ongoing care.

Responsibilities include outbound/inbound calls, RPM alert response, and accurate documentation in the clinical platform. Strong English/Spanish bilingual skills required.

Qualifications

• Active LVN/LPN license required.
• Proficient with EMRs and telehealth tools.
• Strong communication and organizational skills.
• Bilingual proficiency: English and Spanish.

Responsibilities

• Make outbound calls to check in on patients and address health concerns (70–90 calls/day).
• Handle inbound calls and route based on clinical urgency.
• Route non-clinical calls to appropriate departments.
• Monitor RPM alerts and escalate as needed.
• Coordinate with providers for timely patient care.
• Perform monthly wellness assessments and chart reviews.
• Document all patient interactions in the clinical platform in real time.
• Meet or exceed care quality, engagement, response time, and protocol adherence metrics.
