Job summary

PharmD Live in Colorado is seeking a bilingual Spanish-speaking Licensed Practical Nurse (LPN) to support Chronic Care Management and Remote Patient Monitoring programs. This remote role offers flexible hours and the opportunity to deliver patient-centered care, education, and support to diverse patient populations.

You will collaborate with physicians, pharmacists, and care teams, perform comprehensive assessments, educate patients in English and Spanish, document in the EHR, and help enroll

Qualifications

• Must be a licensed LPN in Colorado.
• Fluent in Spanish and English, written and spoken.
• Minimum 2 years in patient care or care coordination.
• Familiar with CCM/RPM programs is a plus.

Responsibilities

• Conduct comprehensive assessments of patients’ physical, emotional, and social health needs.
• Collaborate with physicians, pharmacists, and care teams to develop and update individualized care plans.
• Educate and engage patients in both English and Spanish on chronic disease management, medication adherence, nutrition, lifestyle changes, and self-monitoring.
• Coordinate care across healthcare providers, ensuring referrals, follow-ups, and test results are managed efficiently.
• Monitor RPM data for trends or risk signals and escalates concerns as needed.
• Document patient interactions accurately in the EHR, ensuring compliance with CMS guidelines for CCM and RPM.
• Assist in enrolling eligible patients into CCM and RPM programs and support continuous program improvement.
