Job summary

Alignment Health is seeking an RN Case Manager SNP with a California RN license to manage care for members with chronic and complex conditions. You will coordinate care, perform comprehensive assessments, and collaborate with the care team to close gaps in care and optimize outcomes.

The role emphasizes strong communication, education, and problem-solving skills, with a schedule aligned to Pacific Time (8:00am–5:00pm). Expect dynamic, team-based care delivery and PHI protection.

Qualifications

• Must have active RN license in California (Non-Compact).
• Post high school coursework to obtain RN licensure or AS in Nursing.
• BSN or Bachelor's degree preferred.
• Strong understanding of community resources, home health, and chronic conditions.

Responsibilities

• Coordinates care for member, family, and physician.
• Facilitates referrals/authorizations within benefit structure.
• Completes comprehensive assessments within scope of practice.
• Collaborates with care team to implement plan of care.
• Interfaces with PCPs, hospitalists, NPs, and specialists.
• Provides education and self-management support.
• Assists in problem solving with providers, claims or service issues.
• Works with delegated or contracted providers to coordinate care.
• Maintains confidentiality of PHI in compliance with law and policy.
