Village Care is seeking a Full Time RN Care Manager located in the New York Tri-State area. The role combines care planning, coordination, and assessments to ensure high-quality patient care while supporting hospital and community transitions.

Excellent communication and problem-solving are required, with bilingual proficiency in Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole highly valued. Hybrid work arrangements apply.

Qualifications

• Valid NYS RN license and active registration.
• Associate's degree or higher required.
• 3+ years of experience in Managed Care, Home Care, or community settings.
• Strong communication and problem-solving skills; bilingual ability is essential.

Responsibilities

• Develop and manage care plans and coordinate services.
• Monitor quality and progress toward individual goals.
• Oversee transitions and discharge planning from hospitals or nursing homes.
• Identify health, environmental, and psychosocial risks and implement interventions with the care team.
