Elevance Health is seeking a Nurse Care Manager I in a remote-capable role, based in Puerto Rico with in-person training as needed. The position collaborates with providers to manage care for members with complex needs and aims to improve health outcomes through tailored care plans.

Required are a nursing degree and active RN license with multi-state eligibility, plus 3+ years of acute care experience. Bilingual in English/Spanish is strongly preferred to serve a diverse member base.

Qualifikationen

• HS diploma or equivalent with at least 3 years of acute care clinical experience.
• Current, active unrestricted RN license in applicable state(s) required.
• Multi-state licensure required for services in multiple states.

Aufgaben

• Collaborates with providers to assess consumer needs for early discharge planning.
• Conducts clinical assessments and develops goals to implement care plans.
• Monitors and evaluates the care management plan and adjusts as needed.
• Assess inpatient and discharge needs to ensure appropriate, high-quality care.
• Interprets care appropriateness with clinical reviewers or medical directors.
