Job summary

Urban Health Plan is seeking a Referrals Specialist I for a hybrid Bronx-based role. The position focuses on patient information management, insurance verification, and referrals, with experience using EMRs such as ECW, EPIC, and EQUIPO.

The schedule is 40 hours per week, Monday–Friday, 8:00 am–5:00 pm, with three days onsite and two days remote. Full-time benefits include comprehensive health insurance for you and family, dental, vision, life insurance, and a 401(k).

Qualifications

• High School Diploma required.
• 1–2 years of related experience in health care with medical terminology and coding.
• Bilingual Spanish/English required.
• Excellent communication and customer service skills.
• Microsoft Office Proficient.

Responsibilities

• Inputs high volume of patient information and referrals via phone and fax.
• Reviews patient info for specialty, referral reason, ICD-10, medications, etc.; maintains EMRs (ECW, EPIC, EQUIPO).
• Assists with managing reception calls and referral action steps.
• Sought prior authorization from patient insurance carriers.
• Responds to patient information requests and schedules timely follow-ups.
• Ensures confidentiality compliance.
• Contacts external specialists and arranges patient appointments.
