Urban Health Plan is seeking a bilingual Specialty Case Manager to support patients at Bronx health centers within the Social Services department, focusing on behavioral health. The role includes social services, referrals, advocacy, and identifying needed supports for patients.

The position is onsite in Bronx, NY, with hours 8:00 am–5:00 pm, Monday to Friday. Entry level; Spanish/English bilingual required and proficiency in Microsoft Office.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Human Services, or related field.
• Bilingual Spanish/English, required.
• Entry level case management position; no prior experience beyond Bachelor's Degree.
• Team-player and willing to go the extra mile to support patients.
• Proficiency with Microsoft Office.

Responsibilities

• Provides case management assistance to patients including populations as assigned.
• Provides letters to patients including letters for public assistance, housing IRS, schools.
• Supports completion of prior authorizations.
• Provides community based resources to the population based on site/ department/ program need.
• Advocates on patients behalf to obtain specific resources or services including specific or specialized services.
• Works closely with departmental coordinators and team members to provide patients with support.
