Job summary

August Rose Health Center is seeking an Adult PRP Counselor to support children, adolescents, and families in community settings. You will help clients develop daily living and social skills, manage emotions, and navigate community resources.

Telehealth options may be used, and collaboration with therapists ensures aligned treatment goals. The role emphasizes active client engagement in homes and communities, documenting progress, and meeting Medicaid standards.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s degree in Human Services preferred.
• Experience in behavioral health or PRP helpful.
• Bilingual Spanish is a plus.

Responsibilities

• Deliver PRP interventions in community, home, school, or telehealth.
• Document progress and maintain compliance with Medicaid standards.
• Collaborate with therapists and care teams for treatment planning.
• Provide skill-building in emotional regulation, daily living, and social skills.
• Assist with transportation, appointments, and resource navigation.
