Job summary

Long Island Cares, Inc. in Town of Smithtown, NY is seeking a 1115 Waiver Program Coordinator. This full-time, 40-hour-per-week role reports to the VP of Finance & Administration and is grant-funded through March 2027.

Responsibilities

• Conduct initial intake/assessment with members.
• Review referrals and confirm member insurance status and enrollment in appropriate program.
• Put together meal boxes according to client’s assessed needs.
• Schedule delivery of meal boxes to client’s homes or LIC satellites for pickup.
• Determine eligibility for cooking supplies and fulfill authorized distributions.
• Keep detailed records of client assessments and deliveries in database.
• Collect and retain proof of delivery documentation.
• Submit appropriate paperwork for reimbursement.
• Oversee invoice approval and payment.
• Perform other duties as assigned by the Director of Nutrition & Community Health Programs and VP of Network Relations and Capacity Building.
