Job summary

Coordinated Care Network seeks a Specialty Care Representative - Bilingual in Fort Lauderdale to manage a caseload of diverse patients, coordinate care with pharmacists and providers, and support medication adherence and access.

The ideal candidate will be fluent in English and Spanish, hold a Pharmacy Technician license, or be willing to pursue trainee registration, with healthcare experience and strong communication skills.

Qualifications

• Effective verbal and written communication in English and Spanish is required.
• Must be licensed (unrestricted) and registered as a Pharmacy Technician or be willing to apply for a temporary registration as a Pharmacy Technician Trainee in the state of employment.
• Bachelor’s degree in social work is preferred; HS diploma or equivalent is required.
• Two years of health care or pharmacy experience is preferred.
• Experience with HIV/AIDS or at-risk patients is preferred.
• Strong interpersonal and organizational skills are essential for this role.

Responsibilities

• Manage a caseload of 350-400 patients from diverse populations living with mental health issues, substance use, HIV, or related psychosocial stressors.
• Contact patients monthly to ensure adherence to medications and care management.
• Build relationships with patients through empathy, active listening, courtesy, and understanding.
• Serve as liaison between patients and healthcare team, triaging medical, psychological, and social issues with pharmacists, doctors, clinics, or providers.
• Collaborate with providers to overcome barriers to care.
• Process medications for fulfillment, sort queued items, and match refills for processing.
• Record all communications with patients, staff, clinics, doctors and insurance agencies.
• Adhere to policies for Specialty HIV program; perform other duties as assigned.
