CCN Pharmacy seeks a dedicated professional to manage care for diverse patients, including those living with HIV/AIDS. You will support medication adherence, liaise with the healthcare team, and help navigate barriers to care.

The role emphasizes interdisciplinary collaboration, strong communication, and meticulous data handling. Travel up to 15% within and out of state may be required, and a valid pharmacy technician registration is essential.

Qualificações

• Effective verbal and written communication skills in English and Spanish.
• Must be licensed (unrestricted) and registered as a Pharmacy Technician or willing to apply for temporary registration as a Pharmacy Technician Trainee in the state of employment.
• High School Diploma or equivalent required; Bachelor’s degree in social work preferred.
• Two years of experience in a health care or pharmacy setting preferred.
• Experience working with clients living with HIV/AIDS or at-risk patients.
• Strong interpersonal skills to affect positive outcomes.

Responsabilidades

• Manage a caseload of 350–400 patients from diverse backgrounds.
• Contact patients monthly to ensure medication adherence.
• Serve as a liaison between patients and healthcare team.
• Collaborate with providers to overcome barriers to care.
• Process medications for fulfillment and match refills.
