Job summary

NeueHealth seeks a Care Coordinator to promote quality, cost-effective outcomes by coordinating across care settings and advocating for members’ needs. The role focuses on patient-centered care throughout the care continuum.

The ideal candidate has at least 2 years in medical management, bilingual English/Spanish, and preferably Medical Assistant certification. This role is based in Leesburg, FL within our clinic network, offering opportunities to impact populations through coordinated services.

Qualifications

• High School Diploma required.
• Minimum 2 years of experience in medical management.
• Bilingual English/Spanish required.
• Certification as a Medical Assistant preferred.

Responsibilities

• Collect relevant clinical data to support the care planning process.
• Support and intervene in Care Plans to promote self-management.
• Collaborate across providers and settings to ensure quality outcomes.
• Provide transition of care interventions as required.
• Coordinate care, discharge planning, and health education as directed.
• Connect members with community resources addressing social determinants of health.
• Adjudicate referrals according to defined business rules across care settings.
• Adhere to Policies and Procedures of the Quality Management Committee.
