Tapestry 360 Health is seeking a Community Health Coordinator for Reproductive Health in Chicago. The role focuses on promoting access for perinatal patients, coordinating care, and bridging language and cultural gaps between patients and providers.

You will support referrals, accompany patients to appointments, and document Plan of Care progress in EMR, while training others and promoting patient independence.

Qualificações

• High School Diploma or GED; 2 years in community programs or health care.
• Proficient in English; bilingual in Spanish.
• Strong cultural humility and interpersonal skills.

Responsabilidades

• Provides inclusive language support to perinatal patients who require languages other than English.
• Connects patients to care coordination resources and navigation services.
• Links prenatal patients to resources such as cribs, car seats, and more.
• Coordinates care with WIC services and transmits messages to care teams.
• Ensures patient understanding of plan of care and follow-up.
• Recruits and leads for group visits as cultural navigator; documents in EMR.
• Assists with referrals, accompanies patients to appointments, and facilitates procedures.
• Creates Plans of Care for higher-needs patients and follows up with documentation.
