Job summary

KidsCare Home Health is seeking a licensed Physical Therapist to provide pediatric home health services. You will evaluate, plan, and administer medically prescribed PT for children in their homes, guiding families in home exercise programs and discharge planning.

Qualifications

• Master’s Degree in Physical Therapy is required.
• Current state-issued professional license.
• Valid state driver’s license.
• CPR certification required.
• Pediatric clinical experience preferred.
• Home health experience preferred.

Responsibilities

• Evaluate and conduct pediatric home-based physical therapy as prescribed.
• Train caregivers in home programs and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and goals.
• Refer patients to appropriate services and resources as needed.
• Maintain supplies and equipment and participate in QA activities.
