Responsibilities

• Contact patients by phone to set up appointments and provide reminders.
• Collaborate with patients, providers and care team to set care goals.
• Provide navigation for patients and address scheduling obstacles to avoid cancellations.
• Address insurance concerns by connecting patients with resources and team members.
• Provide culturally sensitive services to patients from diverse cultures.
• Maintain regular communication with patients’ providers.
• Refer to internal case management services when needed.
• Document interventions in medical records.
• Participate in evaluation and research activities as needed.
