Qualifications

Responsibilities

• Contact patients by phone to schedule appointments and provide reminder calls
• Coordinate with providers and care team to set goals for patient care
• Provide navigation and address concerns to prevent appointment cancellations
• Address insurance concerns by connecting patients with resources
• Maintain regular communication with patients' providers and document interventions
• Refer to internal case management when needed
• Support evaluation and research activities in the program
• Perform other duties to support department objectives
